Ok, first of all, I can't believe you haven't seen the movie. You have to get your hands on it and watch it. Second of all, literally...a river runs through the valley of the mountain where they go fly fishing; figuratively, the river runs through life, is what connects them and gives meaning to life. If you watch the movie, I promise you'll pick up on it.
